dc.description.abstractBakgrunn og hensikt: Den store økningen i antall eldre som lever lenger med funksjonsnedsettelser, gir utfordringer for velferdsstaten. Problemstillingen er hvordan samhandlingsutfordringer knyttet til informasjonsflyt, kultur og tillit kan påvirke reinnleggelser og om statens virkemidler kan styrke disse relasjonelle forholdene. Teoretisk rammeverk er samstyring, virkemidler i samhandlingsreformen og teori knyttet til informasjonsutveksling, kultur og tillit. Metode: Kvalitativt design er brukt for å belyse oppgavens problemstilling. Oppgaven er en case studie. Fokusgruppeintervjuer er brukt for å få frem mest mulig informasjon om samhandlingsutfordringer. Tre fokusgruppeintervjuer, alle tre tverrfaglig sammensatt av fagpersoner fra kommuner og sykehus og et intervju med en brukerrepresentant, har vært med å gi svar på problemstillingene. Resultat: Studien viser at det er samhandlingsutfordringer knyttet til informasjonsflyten ved utskrivning og innleggelse. Ulike virkelighetsoppfatninger og kartleggingsmetoder ved beskrivelse av pasientens funksjon i kommunene og på sykehus viser kulturforskjeller. Informantene fra kommunene har uttrykt mistillit til sykehusets vurdering av de utskrivningsklare pasientene og sykehuset mangler tillit til kommunenes kompetanse til spesialiserte oppgaver. Studien viser at de nettverksbaserte virkemidlene som nettverksmøter og gjensidig hospitering vil ha positiv betydning for informasjonsflyt, kultur og tillit som er viktig for å styrke samhandlingen. Bedre planlegging av helse- og omsorgstjenestetilbudet i kommunen og mer utnyttelse av øyeblikkelig-hjelp sengene kan vise seg å ha god påvirkning på re-innleggelser. En felles samhandlingssjef for kommuner og helseforetak med styringsrett, kunne gitt mulighet for å gjennomføre raskere og mer helhetlige løsninger på samhandlingsutfordringene. Konklusjon: Samarbeidsavtalene har hatt betydning for samhandlingen. Samhandlingsreformen har fremtiden for seg, men det må skapes enda bedre måter å samhandle på for å redusere omfanget av re-innleggelser.language
dc.description.abstractBackground and Objectives: The large increase in elderly who are living longer with impairments provides serious challenges to the welfare state. The question is how challenges in cooperation related to the flow of information, differences in institutional culture, as well as mutual trust are influencing re-admissions, and if the tools of government are able to strengthen these relational conditions. The theoretical framework is related to governance and the effect of the National Cooperation Reform, as well as cultural factors and trust issues. Method: A qualitative design was used in this study. The subject is a case study. Group interviews were performed to obtain the maximum amount of information concerning the challenges related to communication. Three interviews were performed with interdisciplinary personnel working in the community and working in the hospital. One patient who had been involved in both systems were also given an opportunity to discuss the problems and challenges experienced. Results: The study shows that there are many challenges related to the flow of information concerning hospital admission and discharge. Difference in perception of patient’s conditions, functional levels and mapping methods between communities and hospitals shows relevant differences in culture between these two organizations. The care team working in the community had a lack of confidence in the ability of the specialist hospital team to assess the suitability of a patient to be discharged. The specialist team in the hospital had a lack of confidence in the ability of the community care team to perform skills and tasks required. The study demonstrates that improving network-based strategies, such as network meetings and mutual job- shadowing, could have a positive impact on improved exchange of information, cultural understanding and trust, thereby enhancing overall cooperation. Better community health care planning and the use of emergency beds can also have a positive influence on readmissions. A coordination consultant with powers to act on behalf of both the specialists in the hospital and the employees in the community health care team could result in a more effective solution to mutual challenges concerning the best interests of the patient. Conclusion: Agreements to effectuate collaboration have shown a positive effect on inter - departmental co-operation. To promote greater effect of The National cooperation reform better ways of cooperation is essential, in order to reduce re-admissions.language
