Fra fortellerkultur til koding. En kvalitativ studie om sykepleieres erfaringer med
sykepleiedokumentasjon i DIPS behandlingsplan
Formål Den overordnede hensikten er å studere sykepleieres erfaringer med en
behandlingsplan der klassifikasjonssystemene North American Nursing Diagnosis
Association (NANDA) og Nursing Intervention Classification (NIC) er integrert. De konkrete
formål er å (1) studere hvordan sykepleierne opplever det språklige uttrykket i disse
systemene, (2) utforske erfarte fordeler og ulemper ved sykepleieklassifikasjonene og (3)
identifisere hva som fremmer og hemmer mestring ved bruk av behandlingsplan.
Bakgrunn I 2014 ble det elektroniske pasientjournalsystemet DIPS innført ved Oslo
Universitetssykehus (OUS). DIPS medførte behandlingsplan med NANDA og NIC integrert
som hoveddokument i pasientens sykepleiedokumentasjon.
Metode Studien er deskriptiv og har et kvalitativt og utforskende design. Det er utført en
innholdsanalyse av data fremkommet etter dybdeintervju og fokusgruppeintervju med ni
sykepleiere. Studien ble gjennomført ved Kreftklinikken, OUS.
Resultater Studiedeltagerne opplevde ofte språket i NANDA og NIC som unaturlig. Enkelte
klassifikasjonskoder ble oppfattet som uforståelige. Sykepleierne betraktet bedret oversikt,
økt planmessighet og mindre behov for fortløpende notater som fordeler. Klassifikasjonene ga
lite rom for nyanser, og informantene uttrykte behov for å supplere med muntlig rapport.
Oppfølging fra superbrukere og tilgjengelige veiledende behandlingsplaner var
mestringsfremmende faktorer. Utilstrekkelig opplæring og tidsmangel opplevde
respondentene som hemmende for mestring.
Konklusjon Det er hensiktsmessig å inndele sykepleiernes erfaringer av endringene i
praktiske og prinsipielle kategorier. Den praktiske kategorien omfatter arbeidsflyten i den
kliniske hverdag og opplevelsen av teknostress. Den prinsipielle kategorien omfatter en
dreining av dokumentasjonen fra fortellende beskrivelser til en standardiserende og
klassifiserende kommunikasjonsplattform. Studien er et bidrag til å forstå hvordan
organisatoriske endringer fundamentalt kan forandre sykepleiens dokumentasjonsmåter,
samtidig som endringene påvirker den enkelte sykepleiers arbeidshverdag. Title

Purpose The aim of the study is to gain knowledge of nurses' experiences, using the
classification systems North American Nursing Diagnosis Association (NANDA) and
Nursing Intervention Classification (NIC) in the nursing care plan. The specific purpose is to
(1) examine nurses’ experience of the linguistic expression in these systems, (2) explore the
facilitators and barriers of applying nursing classifications, and (3) identify the promotional
and inhibitory factors of coping with the nursing care plan.
Background The electronic health record system DIPS was introduced at the Oslo University
Hospital in 2014 with the care plan as the main nursing document. The DIPS integrated the
classification systems NANDA and NIC as parts of the nursing care plan.
Methodology The study is descriptive, and has a qualitative and exploratory design. Content
analysis of the data has been conducted, based on focus group interviews and in-depth
interviews of nine nurses on a cancer ward.
Results The participants perceived the formulations in the classifications as unnatural. Some
classifications were considered incomprehensible. The classifications facilitated the overview
and the availability as well as reduced the need for nursing notes. The reported barriers were
fixed formulations with less space for individual expressions, and the informants experienced
a need for supplements of verbal handovers. Promotional factors were the follow-ups by the
superusers and easy access to standardized nursing care plans. Inhibitory components seemed
to be time shortage and incomplete instructions prior to start.
Conclusion The nurses’ experiences of the care plan can be divided into two categories. The
practical category comprises the change of the nurses’ clinical workflow and the experience
of technostress. The principled category comprises the shift from a narrative tradition to a
prospective planning paradigm with a standardized nursing classification. This study can
contribute to the understanding of how organizational changes may affect the nursing
documentation, which also interferes with the everyday nursing practice.
