Samarbeid, demokrati og kvalitet : en dekonstruktiv lesing av Veileder for poliklinikker i psykisk helsevern for barn og unge

Sammendrag
Denne masteroppgaven handler om samarbeid mellom terapeut og pasient i psykisk helse for
barn og unge. Feltet psykisk helse har vært og er fortsatt preget av to tendenser i dagens
Norge. Den ene har fokus på demokratisering og økt pasientmedvirkning, den andre har fokus
på effektivitet, kvalitet og forskningsbasert grunnlag når det gjelder å yte helsehjelp.
Samarbeid for barn, unge, deres familier og behandlere, sett fra et sosialkonstruktivistisk
ståsted, er noe som konstrueres og reproduseres i den sosiale samhandlingen mellom
deltakerne og gjennom de eksisterende praksiser som institusjoner preges av. Barn og unge er
pasienter som befinner seg i en særlig sårbar posisjon, siden de er avhengige av sine
omgivelser, først og fremst sine foreldre. I denne oppgaven har jeg valgt å undersøke hvordan
samarbeid for barn og unge i feltet psykisk helse blir omtalt i en sentral retningsgivende tekst
utarbeidet av Helsedirektoratet: Veileder for poliklinikker i psykisk helsevern for barn og unge.
Jeg har valgt dekonstruksjon som hovedtilnærming, både teoretisk og metodisk. Jeg har hatt
til hensikt med denne tilnærmingen og ved hjelp av noen diskursiver innganger, å se etter
hvordan samarbeidet blir skrevet frem, og hvordan dette får konsekvenser for posisjonene
som er tilgjengelige for de involverte. Noe i samarbeidet er fraværende, noe er tilstede. I min
analyse synes det at veilederen er preget av to motstridende diskurser: den demokratiske og
den medisinske, som også preger feltet psykisk helse for øvrig . Analysen viser hvordan disse
diskursene posisjonerer barn, familier og behandlere i samarbeidsøyemed. This thesis is about the collaboration between therapist and patient in the mental health of
children and adolescents. Mental health field has been and continues to be characterized by
two tendencies in contemporary Norway. One focuses on democratization and increased
patient participation, the other focuses on efficiency, quality and research-based foundation
when it comes to providing health care.
Working together for children, young people, their families and clinicians, from a social
constructionist perspective, is something that is constructed and reproduced in the social
interaction between the participants and through the existing practices that institutions are
characterized by. Children and young patients are in a particularly vulnerable position, since
they are dependent on their environment, particularly their parents. I have in this paper chosen
to examine how cooperation for children and young people in the mental health field are
featured in a central normative text prepared by the Directorate of Health: Guidelines for
outpatient mental health care for children and adolescents.
I opted for deconstruction as main approach, both theoretically and methodologically. With
this approach and using discourse entrances, I have had the intention to look for how
collaboration has been express, and how this will impact the positions that are available to
those involved. Something in the cooperation is absent and something is present. In my
analysis it seems that the supervisor is characterized by two contradictory discourses: the
democratic and the medical that also characterizes mental health field otherwise. The analysis
shows how these discourses position children, families and clinicians.
