dc.description.abstractBackground: The municipal acute unit is the municipality's statutory alternative to avoid admission to hospital for patients who need immediate help, but where admission to the specialist healthcare service is not necessary or desirable. As an addition to an already complicated and fragmented healthcare service, communication and interaction with other actors in the municipal healthcare service is a challenge. The challenge is particularly distinctly in the healthcare provider`s communication regarding elderly multimorbide Method: The study is based on semi-structured interviews with 20 healthcare providers in collaborative healthcare services in the municipality. The data was analyzed using Braun and Clarke's thematic analysis. Results: Three main themes manifested: 1) Communication goes through a coordinator, 2) communication with nursing- and care messages is not used as intended and 3) the routine for communication at discharge is weak. Conclusion: Even with good communication framework to ensure quality interaction and that information about the patient reaches the right healthcare providers, it varies whether and how these are used and understood. Many people are involved, which challenges the continuity of information. On discharge, the healthcare providers experience that information is unclear and they find alternative solutions to get correct information within the existing framework and structures.en_US
