Objective: Musculoskeletal pain (MSP) and anterior cruciate ligament injuries are increasing in number among adolescents. The purpose of this study was to investigate whether MSP is associated with a future anterior cruciate ligament reconstruction (ACLR) and if the risk of ACLR differ between upper and lower body pain. Methods: This study is a population-based prospective cohort study. 8087 adolescents in former Nord-Trøndelag county in Norway were included. The exposure was self-reported MSP from the Young-HUNT3 (2006-2008) study categorized into “load”-groups based on frequency and number of pain sites, and upper and lower body pain based on anatomical localization. The outcome was ACLR recorded in the National Knee Ligament Registry between 2006 and 2019. Logistic regression was used to estimate the association between MSP and ACLR, given as odds ratios (ORs) with 95% confidence intervals (CIs). A p value of ≤0.05 was considered statistically significant. Results: 99 ACLRs were identified. Mean age at baseline was 15.6 years. ACLR occurred in six cases (0.9%) in adolescents with high MSP load and in 93 cases (1.3%) in adolescents with low MSP load (OR 0.71, 95% CI 0.28, 1.76). Both groups with MSP in the upper (OR 0.66, 95% CI 0.37, 1.13) and lower body decreased odds of a future ACLR (OR 0.87, 95% CI 0.43, 1.76). None of the results were statistically significant. Conclusion: MSP in adolescents was not significantly associated with the risk of future ACLR. Future studies should be sufficiently powered to establish more reliable risk estimates of the significance of MSP for future ACLR.en_US
