Objective: The objective was to investigate whether the Canadian Occupational Performance Measure (COPM) was a suitable assessment in an outpatient stroke clinic, and to investigate if the COPM could give information about hidden impairments like fatigue, reduced awareness and acceptance of disability when combined with activities of daily living (ADL). Method: A prospective longitudinal study was conducted using semi-structured COPM interviews at 3 and 12 months post-stroke. Changes in activity status from 3 to 12 months were measured. The patients’ experiences were recorded and summarized. Analyses were performed using descriptive statistics and content analysis. Results: Sixty patients were included, 44 patients had problems with ADL at 3 months post-stroke, 22 of these still had problems at 12 months. Twenty-six patients experienced fatigue and 24 experienced cognitive impairments at 3 months. At 12 months, 19 patients still experienced fatigue and 19 experienced cognitive impairments. Sixteen patients improved from 3–12 months after stroke, the situation for 19 patients was unchanged, none deteriorated. Conclusion: To our knowledge, the COPM gives good information about activity and participation, as well as information about fatigue and cognition. The COPM gives little to no information about awareness and acceptance of stroke impairment for this group of patients.en
