dc.description.abstractBakgrunn: Vurdering av pasientens anestesidybde er en sentral oppgave for anestesisykepleier. Vi har erfart at holdningene til bruk av Bispectral index (BIS) og hvordan BIS-verdiene tolkes i samspill med det kliniske blikk og kliniske variabler varierer i stor grad, til tross for eksisterende retningslinjer. Problemstilling: Hvilke holdninger har anestesisykepleiere til bruk av anestesidybdemonitoren BIS til pasienter i generell anestesi? Hensikt: Utforske og kartlegge hvilke aspekter som ligger til grunn for anestesisykepleieres holdninger til bruk av BIS. Teoretisk forankring: Tidligere forskning som har fokusert på holdninger til bruk av anestesidybdemonitorering, viser en endring i anestesiologers bruk av denne. Nyere studier viser større tiltro til monitoren for vurdering av anestesidybde, sammenlignet med eldre studier. Trekomponentmodellen tar utgangspunkt i at holdninger består av en affektiv, kognitiv og atferdsmessig komponent. Anestesisykepleiers ansvars- og funksjonsområder består blant annet av fagutvikling, forebygging av komplikasjoner og anvendelse av teknologi. Metode: Dette er en kvalitativ studie ved bruk av fokusgruppeintervju. Vi har utført en pilotstudie for å utforske om metoden er gjennomførbar og for å samle erfaring som moderator og sekretær. Seks anestesisykepleiere deltok i studien. Resultater: Majoriteten av anestesisykepleierne brukte BIS jevnlig, spesielt ved totalt intravenøs anestesi og samtidig bruk av muskelrelaksantia. BIS-verdiene ble i noen tilfeller ansett som upålitelige ved inhalasjonsanestesi, ulike typer leiring av pasienten og anestesi til barn, og ble derfor mindre brukt i disse situasjonene. Til tross for positiv holdning til BIS, uttrykket anestesisykepleierne en mangel og et ønske om økt dybdeforståelse av monitoren. Konklusjon: Informantenes holdning til bruk av BIS var avhengig av tallverdiens pålitelighet, deres kunnskap om BIS monitorens funksjoner, dens begrensninger og nytteverdi. På bakgrunn av våre funn synes det fornuftig å anbefale videre forskning på anestesisykepleieres holdninger til bruk av BIS. Studien viser også at det er en etterspørsel blant anestesisykepleiere om mer kunnskap og ferdigheter i bruk av BIS.en
dc.description.abstractBackground: Evaluating the patient’s depth of anesthesia is a central task for a nurse anesthetist. We have experienced that the attitudes towards the use of Bispectral index (BIS), and how BIS-data are interpreted in an interplay with the clinical observations and clinical variables varies to a large extent, in spite of existing guidelines. Research question: Which attitudes do nurse anesthetists have towards use of the depth of anesthesia monitor, BIS, for patients in general anesthesia? Objective: To explore and map the aspects affecting nurse anesthetists’ attitudes to the use of BIS. Theoretical foundation: Previous research regarding attitudes to the use of depth of anesthesia monitoring, shows that there is a change in anesthesiologists using patterns. Newer studies show a greater acceptance towards the monitor compared to previous studies. The three-component model consider attitudes to consist of an affective, cognitive and behavior component. Nurse anesthetists´ responsibility and function are among others; professional development, prevention of complications and use of technology. Method: This is a qualitative study by means of a focus group interview. We performed a pilot study to determine whether the method was feasible and to gain experience as moderator and secretary. Six nurse anesthetists´ participated. Results: Most of the nurse anesthetist´ used BIS on a regular basis in clinical practice, especially during total IV anesthesia with muscle relaxants. In some cases, the BIS data was considered less reliable such as in use of volatile agents, some surgical positions and during pediatric anesthesia, and therefore used less in these cases. Despite positive attitudes towards BIS, the nurse anesthetists´ felt a lack of, and a wish for more in-depth knowledge of the BIS monitor. Conclusion: The informants’ attitudes towards the use of BIS, depended on the reliability of the data supplied by BIS, their own knowledge about the different options provided by BIS, and its limits and utility value. Based on our findings, it seems that there is a rationale for recommending use of BIS. Further research on nurse anesthetists´ attitudes to BIS is warranted. The study also shows a demand among nurse anesthetists´ for more knowledge and skills in using BIS. Key words: Bispectral index, monitor, nurse anesthetist, attitudes, knowledgeen
