«Å føle seg velkommen har mye å si» -en kvalitativ studie om hvordan eldre med kommunal rehabiliteringsplass opplever kommunikasjonen med personalet i et empowerment perspektiv.

Abstract

For å kunne løse fremtidige utfordringer i forhold til et stort antall økende eldre og deres behov for rehabilitering, er det viktig å få mere kunnskap om hvordan helsepersonell kan kommunisere med eldre på en god måte. Det finnes i dag lite kunnskap om eldres subjektive opplevelse av kommunikasjon i denne sammenhengen. Målet med denne studien er å undersøke hvordan eldre med kommunal rehabiliteringsplass opplever kommunikasjonen med personalet. Studien har en kvalitativ, fortolkende design med intervju og observasjon som metode og er forankret i hermeneutisk og fenomenologisk tradisjon. Analysen er utført ved hjelp av systematisk tekstkondensering. En relasjonell forståelse av kommunikasjon og et empowerment perspektiv er benyttet for å fortolke dataene. Studien viser hvordan seks eldre mennesker med kommunal rehabiliteringsplass opplever å kommunisere med personalet. Funnene kan i hovedsak beskrives som opplevelsen av å bli forstått eller ikke å bli forstått. Flere faktorer som fremmet opplevelse av god kommunikasjon ble identifisert; tillit til personalet, å føle seg sett og hørt, å motta forståelig informasjon og tilstrekkelig tid til å kommunisere. Faktorer som hindret kommunikasjon var sykdom og redusert kapasitet, og dette ble av informantene oppgitt som årsak til ikke å bli forstått. Ulike forventninger til rehabiliteringsprosessen mellom informantene og personalet førte også til at informantene ikke følte seg forstått. Informantene fortalte om sine følelser i form av å beskrive hvordan de opplevde å være mere sårbare. De viste også mange følelser under intervjuene. Denne studien viser at følelsesmessig samstemthet var avgjørende for informantenes opplevelse av å ha en god og bemyndigende kommunikasjon med personalet.

To solve future challenges concerning the growing number of old people and their need for rehabilitation it is important to gain more knowledge of how health care professionals can communicate with older people in a good way. There is currently little knowledge about older people's subjective experience of communication in this context. The aim of this study is to investigate how older people in a community based rehabilitation unit experience the communication with the healthcare professionals. The study has a qualitative, interpretative design. Data collection entailed observations and interviews and has a hermeneutical/phenomenological approach. The interviews are analysed by systematic text condensation. To interpret the data, a relational understanding of communication and a perspective of empowerment has been used. This study shows how six older people in a community based rehabilitation unit experiences to communicate with the staff. The results mainly describe the experience of being understood or not being understood. Several factors for promotion of good communication was identified; trust in staff, the feeling of being seen and heard, to receive understandable information and adequate time to communicate. The experience of not being understood was related to reduced capacity in the informant because of illness or other functions related to communication. Different expectations between the informants and the staff concerning the rehabilitation process also led to the experience of not being understood. The informants told about their emotions in the way of describing the experience of being more vulnerable. They also showed many emotions during the interviews. This study shows that emotional coherence was significant for the informants’ experience of having good and empowering communication with the staff.
